Health Insurance Card
Please bring your Insurance card.
If you do not have Japanese Health Insurance card, you need to pay the full amount of the medical fees.
Examination Card
If you have TOBATA KYORITSU HOSPITAL examination/registration card, please bring it with you.
ID
Passport or other photo ID to prove your ID including your birthday.
Referral Letter
If you have a referral letter from another hospital, you need to bring it with you. If you do not have a referral letter, you will be charged for 7,700JPY (tax included) in addition to the first visit fee (2,880JPY).
Interpreter
If you do not speak Japanese, please visit our hospital with your interpreter.
Payment
Payment substitutions are not available. We cannot provide direct billing.
